Participation Waiver.
I agree and understand that Life Waters Ministries International does not assume any responsibility for loss of property, damage to the same, personal harm or illness that may come; and I, for myself, my heirs, executors, administrators, distributes and assigns, in consideration of my willingness to participate in any or all activities do hereby absolve Life Waters Ministries International, and hold them harmless from any claim or demand which I or they might conceivably assert upon the basis of the foregoing. By participating in exercise or physical activities, I attest that I am doing so at my own risk and any questions about my fitness to participate are best addressed by my personal physician prior to participation. In addition, I hereby release Life Waters Ministries International from all claims and liability relating to audio recordings, photographs or video footage taken as a result of participation during this activity. I also grant them the right to use and incorporate (alone or together with other materials), in whole or in part, audio recordings, photographs or video footage taken during this activity. Unauthorized recording, broadcast, or sale of recorded materials from any session of this conference is strictly prohibited. I certify that I am at least 18 years of age or older.
